Completing these forms ahead of time helps reduce wait times and allows Dr. Soni to review your information before your visit.
If you’re unable to complete forms online, please arrive early so we can assist you in the office.

Insurance and Payment Information

If you plan to use dental insurance, please provide your insurance details when completing your forms. We will submit claims on your behalf as a courtesy and provide estimates based on the information you share.
Please remember that insurance estimates are not guarantees of payment. Patients are responsible for all charges incurred, and payment is due at the time of service.
No insurance? No problem.
We offer the Equinox Membership Plan, a monthly dental savings plan designed to help make care more affordable. We also partner with third-party financing companies to provide flexible payment options.
