Adult Annual Membership
$35/Month
$149 due at enrollment, then $35/month for 11 months
or
$499/year billed annually
Renews annually. Pricing subject to the then-current fee structure on your renewal date.

Perio Maintenance Membership
$50/Month
$249 due at enrollment, then $50/month for 11 months

For patients enrolled in active periodontal maintenance due to prior treatment of periodontal (gum) disease. Scaling and Root Planing (deep cleaning), if needed, is charged separately and discounted by 20%.

This is not dental insurance and cannot be combined with dental insurance or other discount plans. This membership is a direct agreement between the patient and Equinox Dental and is honored only at our office. Benefits must be used within the plan year and do not carry over or roll over. Membership is non-transferable. Fees are subject to change at renewal. Need for X-Rays and Non-periodontal or Periodontal Cleaning to be determined by your dentist.
